-S/o Sh. solemnly, affirm & declare as Under:- permanent R/o VPO Do hereby,

1. That date of birth of deponent is, I will complete 55 years age as on ---------and further require extension in service upto the age of 58 years on attaining the age of superannuation of upto whichever is earlier.

2. That the initial appointment of the deponent was on dated_ as JBT/TGT/PGT in (School Name with Code). The deponent currently working as Principal in Government Senior Secondary School, transfer/adjustment/deputation/administrative ground.

3. That the deponent hereby declare that no complaint/enquiry are pending

against him as on date. 4. That the deponent hereby declare that no judicial/departmental proceedings are pending against him as on date.

5. That deponent have never been convicted by any court of law or the

offence punishable under any section of Indian Panel code or violation of

law or regulation. No criminal proceedings pending against the deponent

as on date.

6. That deponent have never been involved/called in any inquiry before State Vigilance Bureau Haryana against him or others as on date.

7. That deponent maintained absolute integrity & devotion to duty and done nothing which is comes under the preview of unbecoming of Government Employee.
